MHC employs a team of qualified professionals to work with purchasers, clients and Home Managers to ensure a smooth transition into our services. Emphasis is placed on undertaking a comprehensive pre-placement assessment of each individual’s needs, and designing a bespoke care package to best meet those needs.
At the point of enquiry or referral the most suitable Placement Officer will be allocated to the individual’s case, and this Officer will then handle all aspects of the individual’s potential admission.
Upon receipt of your documentation, the designated Placement Officer will contact you to arrange an assessment at the earliest opportunity (normally within the first 7 days following receipt of the referral). This assessment will normally be conducted in the service user’s present environment.
Your designated Placement Officer will then remain your key contact throughout the whole process, including admission, to ensure a smooth transition.
The Assessment
MHC uses various assessment tools, including the HoNOS, as part of the assessment process. The views of the service user/Advocates, including family members and key care professionals, are all taken into account in deciding whether an appropriate service can be offered. Following the preliminary assessment, a service proposal with related costings will be forwarded to you.
Once funding has been agreed, the Placement Team will work closely with you and the prospective Home Manager to prepare the full care package, including detailed risk assessments, management strategies and care plans, and to cover the contractual arrangements prior to the individual's admission.
A pre-placement visit, including an overnight stay, is offered where appropriate. There is no charge for these visits.
There are no facilities available for emergency admissions.
Admission into MHC’s ServicesUpon admission into the service the client will undergo a period of assessment within the new environment. This ‘assessment period’ normally takes thirteen weeks. During this time, psychological and psychiatric assessments are undertaken and interventions identified for managing and supporting the client's individual care and rehabilitation. At the end of the initial assessment period a full multi-disciplinary review will be held to discuss and agree future provision.
Making a ReferralYou can make a referral by completing and returning the Application for Placement form, enclosing the following most recent reports:-
Psychology
Psychiatry
Social History
Community Care Assessment
And any other information that may assist the Placement Officer in identifying the most appropriate service.
Your referral will be treated in the strictest of confidence and will be dealt with promptly and professionally at all times.
